| Position Objective: |
- Identify and assess client and family needs of minors placed in care by DCF due to abuse or neglect by caretakers. Evaluate, coordinate and ensure necessary services and/or treatment are provided. Complete required assessments. Assist individuals and families using activities such as delineating alternatives, helping articulate goals and providing necessary information to them.

| Essential Functions May Include: |
- Conduct child safety assessments
- Assess client needs and develop service plans
- Coordinate delivery of services, plan referrals and linkages to client and families
- Monitor service plan progress and evaluate reports from multiple service providers
- Develop and maintain case and program documentation according to contract and agency standards (assessments, treatment/service plans, progress notes, termination summaries and others as requested)
- Ensure services provided comply with contract and licensing requirements and internal quality assurance standards
- Prepare case/status reports and make recommendations regarding termination of parental rights or other permanency plans or testify at court when subpoenaed as witness
- Conduct diligent searches as required by Dependency laws
- Coordinate services with other professionals and paraprofessionals
- Conduct community outreach as needed
- Provide specialized services that my include shelter, relative/non relative caregiver home studies, court testimony, termination of parental rights and judicial review staffing
- Provide support to children, caregivers and family members to assure stable placement and timely achievement of permanency
- Assist family/caregivers in accessing subsidy, medical coverage and other needed services for children
- Coordinate cases with supervisor, team members, providers, DCF and attorneys as needed
- Facilitate placement and independent living
- Arrange for transportation of clients when necessary
- Arrange for drug/alcohol screens as necessary
- Provide on call support as scheduled or required
- Participate in the Quality Improvement process
- Compile required reports and submit to supervisor on timely basis

| Required: |
- Bachelor's degree in human services field from an accredited college or university. Transcripts or diploma required
- Two years experience providing direct services to children, youth and families
- Professional Development Center certification required (eight week PDC training and examination)
- Must complete re-certification requirements every three years
- Must meet level 2 background screening requirements as specified in Chapter 435 Florida statues, employment screening
