POSITION TITLE: Diversion Case Manager
LOCATION: Martin, Okeechobee, Indian River



OBJECTIVES & FUNCTIONS:

Position Objective:
  • Identify and assess client and family needs of minors placed in care by DCF due to abuse or neglect by caretakers. Evaluate, coordinate and ensure necessary services and/or treatment are provided. Complete required assessments. Assist individuals and families using activities such as delineating alternatives, helping articulate goals and providing necessary information to them.
Essential Functions May Include:
  • Conduct child safety assessments
  • Assess client needs and develop service plans
  • Coordinate delivery of services, plan referrals and linkages to client and families
  • Monitor service plan progress and evaluate reports from multiple service providers
  • Develop and maintain case and program documentation according to contract and agency standards (assessments, treatment/service plans, progress notes, termination summaries and others as requested)
  • Ensure services provided comply with contract and licensing requirements and internal quality assurance standards
  • Prepare case/status reports and make recommendations regarding termination of parental rights or other permanency plans or testify at court when subpoenaed as witness
  • Conduct diligent searches as required by Dependency laws
  • Coordinate services with other professionals and paraprofessionals
  • Conduct community outreach as needed
  • Provide specialized services that my include shelter, relative/non relative caregiver home studies, court testimony, termination of parental rights and judicial review staffing
  • Provide support to children, caregivers and family members to assure stable placement and timely achievement of permanency
  • Assist family/caregivers in accessing subsidy, medical coverage and other needed services for children
  • Coordinate cases with supervisor, team members, providers, DCF and attorneys as needed
  • Facilitate placement and independent living
  • Arrange for transportation of clients when necessary
  • Arrange for drug/alcohol screens as necessary
  • Provide on call support as scheduled or required
  • Participate in the Quality Improvement process
  • Compile required reports and submit to supervisor on timely basis

QUALIFICATIONS:

Required:
  • Bachelor's degree in human services field from an accredited college or university. Transcripts or diploma required
  • Two years experience providing direct services to children, youth and families
  • Professional Development Center certification required (eight week PDC training and examination)
  • Must complete re-certification requirements every three years
  • Must meet level 2 background screening requirements as specified in Chapter 435 Florida statues, employment screening

  • Ability to work flexible hours and participate in on call rotation, as required
